Basic Information:

Trip Dates: July 15 - 23 , 2022

Requirements:
Failure to commit to these requirements means CCV has the right to ask you not to participate in the trip. Vaccinations are currently not required, this is subject to change due to the DR's regulations.

• Passport must be valid and in hand by April 1, 2022. Cannot expire before January 2023. 
• Complete a CCV screening background check. 
• Attend CCV's Starting Point class (if you haven't already been)
• Attend all pre-trip preparation meetings.  TBD
• ALL participants must have a negative COVID PCR test result 24 hours prior to leaving the US as well as a negative COVID PCR negative result 24 hours prior to travel back into the US (estimated COVID PCR test $150-250)

Trip Cost: $2500
        Included in this fee: Airfare from Phoenix to Puerto Plata, Dominican Republic, accommodations, transportation and meals while in country.  This also cover medical insurance, luggage fees, Visa fee, and pre-trip materials.

        Not included: Passport fees, vaccinations, and any personal spending money for meals during travel and souvenirs.

$250 deposit per person is non-refundable at the time of registration. If a cancellation is made after airline tickets have been purchased you will be responsible for the cost of the ticket.

Do I need to get any shots? Where can I get shots?

Please visit cdc.gov/travel for up to date vaccination requirements for your destination. Zika virus is also present in the Dominican Republic. There are lots of great travel clinics in the valley to get your shots, so you can find one close and convenient for you. Please inquire and schedule your appointment soon.
